Certificate of Insurance
A certificate of insurance is a document used to verify an entity is insured. It is written evidence that an insurance policy has been issued, indicating both the amounts and the types of insurance. It is used when general contractors or property owners require subcontractors to provide evidence of insurance.
UCAR’s Risk Management & Insurance Office provides the certificate as proof of insurance.
Examples of situations when evidence of insurance may be required:
• Lease or rental agreements for property, vehicles, or equipment furnished to UCAR/NCAR/UOP.
To request a Certificate or Proof of Insurance submit the request for insurance to UCAR’s Risk Management & Insurance Office.

Include:
• Full name and address of the party requiring the Certificate of Insurance
• Fax number
• Event and date, coverage requested, and limits
• Reason that the Certificate of Insurance is required (a copy of the written agreement may be submitted for this purpose)
• Contract/Award Number
• Send instructions for the certificate, including number of copies needed
Insurance Requirements for Independent Contractors and Vendors
UCAR must be named as Additional Insured with respect to General Liability coverage insofar as it pertains to the work done/service provided/product delivered to UCAR/NCAR/UOP. The vendor/contractors policy must be Primary as to any other valid and collectible insurance, but only as to acts of the named insured.
Coverages and limits are to be considered as minimum requirements and in no way limits the liability of the Contractor. A certificate of insurance with the coverage as cited below and listing UCAR as the certificate holder, must be submitted to UCAR Contracts Office before work begins by the Vendor/Contractor. Coverage's represented on the certificates of insurance must show policy numbers, effective dates and limits. All policies shall evidence insurance written by a carrier licensed in the State of Colorado and rated at least “A” in A.M. Best's Key Rating Guide. Renewal certificates of insurance shall be provided annually until the work is completed and for three years following the completion of work. Specific insurance requirements are subject to change dependent upon scope of work.

Professional Liability with a minimum limit of $1,000,000 per claim is required for all professional services including, but not limited to, architects, engineers, consultants, and testing services.
