Travel Accident Insurance
FOREWORD
The University Corporation for Atmospheric Research (UCAR) is the Holder of a policy
of Travel Accident Insurance which helps provide protection for
employees against the hazards of business travel. UCAR pays
the entire cost of this insurance plan.
All employees of UCAR are eligible and are covered anywhere in the world for
accidental injury sustained during the course of any trip made on
behalf of UCAR excluding commuting to and from work and
vacations.
For the purposes of this insurance the trip will be considered as commencing when you
leave your residence or place of regular employment, whichever
you leave last, for the purpose of going on such trip, and the
trip shall continue until you return to your residence or your
place of regular employment, whichever you return to first.
A business trip made on behalf of UCAR for the purpose of this insurance
plan means travel and sojourn authorized by or at the direction
of UCAR for the purposes of furthering the business of
UCAR.
The description of the program given here is for general information only and should
not be relied upon as accurately defining all benefits or all
limitations on such benefits. The applicable benefits, conditions
and limitations are those set forth in the master policy. In case
of discrepancies between this book and the contract, the
provisions in the contract will govern.
AIR TRAVEL COVERAGE
You are also covered anywhere in the world while on a business trip made on
behalf of UCAR and while riding as a passenger and not as a
pilot, operator or member of the crew in any aircraft being used
for the transportation of passengers. In addition you are also
covered anywhere in the world while on a business trip made on
behalf of the Holder and while riding as a passenger or crew
member in any aircraft owned, operated or leased by UCAR
provided such aircraft and crew member have been previously
consented to in writing by the insurance company. If you have
questions regarding coverage in aircraft owned, operated or
leased by UCAR, contact the UCAR Benefits Administrator.
INDIVIDUAL EFFECTIVE DATE
You do not have to enroll. You are automatically covered as of the date you become
an eligible employee of UCAR.
AMOUNT OF PRINCIPAL SUM
All eligible employees are covered for $150,000 principal sum.
AGGREGATE LIMIT
The maximum amount payable for all persons insured under this plan who are involved
in the same accident is $2,000,000, except with regard to
aircraft owned, operated or leased by UCAR. If you have
questions regarding the aggregate limit for such aircraft,
contact the UCAR Benefits Administrator.
ACCIDENTAL DEATH AND DISMEMBERMENT INDEMNITY
When a covered injury results in any of the following losses to an insured
person within 365 days after the date of the accident, the
insurance company will pay in one sum the indicated percent of
the principal sum for:

| Loss of both hands or both feet | 100% |
| Loss of entire sight of both eyes | 100% |
| Loss of one hand and one foot | 100% |
| Loss of one hand or one foot | 50% |
| Loss of entire sight of one eye | 50% |
| Loss of speech | 50% |
| Loss of hearing in both ears | 50% |
| Loss of thumb and index finger of same hand | 25% |
 
"Loss" as above used with reference to hand or foot means the actual,
complete and permanent severance through or above the wrist or
ankle joint; as used with reference to eye means irrecoverable
loss of entire sight thereof; as used with reference to speech
means complete and irrecoverable loss of speech; as used with
respect to hearing means complete and irrecoverable loss of
hearing in both ears; and as used with respect to thumb and index
finger means the actual, complete and permanent severance through
or above the metacarpophalangeal joints. Indemnity provided under
this provision will not be paid, under any circumstance, for more
than one of the above losses, the greatest, sustained by you as
the result of any one accident.
DISAPPEARANCE
100% of your benefit amount will be paid to your beneficiary if you are not
found within one year of the disappearance, sinking, or wrecking
of a conveyance in which you were riding.
EXPOSURE TO THE ELEMENTS
If you are exposed to the elements as the result of an accident and suffer any loss
otherwise covered under the policy, your benefits will be
payable.
DESIGNATION OF BENEFICIARY
That beneficiary or those beneficiaries specifically designated in writing by you for
this insurance and on file with UCAR; otherwise, to that
beneficiary or those beneficiaries specifically designated in
writing by you for UCAR's group Life Insurance policy.
Otherwise, to the first of the following surviving you: Your A)
spouse; B) children; C) parents; D) brothers and sisters, or E)
estate.
INDIVIDUAL TERMINATIONS
Your insurance will terminate on the earliest of the following dates:
- 1)
- the date the insurance policy is terminated;
- 2)
- the date you cease to be associated with UCAR in a capacity making you
eligible for the plan;
- 3)
- the premium due date if UCAR fails to pay the premium,
except as the result of inadvertent error.
EXCLUSIONS
Under this plan you are not covered for:
- 1)
- suicide or any attempt thereat while sane or self- destruction or an attempt
thereat while insane;
- 2)
- declared or undeclared war or any act of either within the United
States, the District of Columbia or Canada;
- 3)
- air travel other than as previously described in "Air
Travel Coverage";
- 4)
- service in the armed forces of any country, however, orders to active military
service for two months or less shall not constitute service in the armed forces;
- 5)
- sickness or disease, except pyogenic infections which occur
through an accidental cut or wound.
This insurance is underwritten by the Continental Casualty Company, Chicago
Illinois on Policy form Pl-66721-A.

plan.
