Form Title: Fixed Asset Capitalization Form
Last Revised: 3/2007
Purpose: This form documents whether the cost of a new purchase, which replaces an existing component, should be capitalized and included in the cost of an existing Fixed Asset.
Who Should Use the Form: Division Property Administrators (DPAs)
Instructions: Use the Fixed Asset Capitalization Decision Tree to determine whether the cost of a new purchase, which replaces an existing component, should be capitalized and included in the cost of an existing Fixed Asset.  If yes, fill in the name of the individual completing the form, the division, the Requisition number for the new purchase, and the property tag number (PPE#) of the existing Fixed Asset.  For each Question, put an "X" next to the appropriate response and follow the instructions to the right of that response to determine whether to continue to the next Question. Send the completed form to the UCAR Property Office.
