| Purpose: | To authorize and document loans of UCAR-accountable equipment to non-UCAR entities. |
| Who Should Use the Form: | Division Property Administrators (DPAs) |
| Instructions: | Fill in all information requested on the form and obtain appropriate signatures, including the UCAR Property Administrator (signs last), before the loan takes place. DPA sends the original form to the UCAR Property Office, retaining a copy for division records. DPA notifies the Property Office by e-mail when the equipment is returned. (Item II.5, optional, completed by division, if appropriate, to specify costs [e.g., recalibration] the division expects to recover from the borrower.) LEGAL DOCUMENT, DO NOT ALTER. CONTACT PROPERTY OFFICE IF CHANGES ARE NEEDED. |
