Purpose:	Allows direct deposit of health care and/or dependent care reimbursements into the employee's designated account.
Who Should Use the Form:	Employees contributing to Health Care and/or Dependent Care Flexible Spending.
Instructions:	Access the WageWorks Web site -- www.wageworks.com First-time users will need to register. Returning users enter their login and password. In the "Manage My Accounts" box on the right, click on "Edit My Profile." Scroll down to the "Reimbursement Payments" box and complete the information required to set up direct deposit. Click "Next," review the profile and then click "Submit." No paperwork is required.
