Purpose: The TIAA/CREF Beneficiary Change Form is used to change the employee's beneficiary(ies) listed on their TIAA/CREF Retirement plan.
Who Should Use the Form: Employee
Instructions: Employee completes the form, signs and mails it directly to TIAA/CREF (address on the bottom of the form). An employee may also log into their account online and change the beneficiary(ies) there.
