Form Title: Position Description - Human Resources
Last Revised: 3/2010
Purpose: The duties section is particularly important for reclassification requests.  It should describe current job duties.  A "Y" should be placed in the last column if the duty is new to the job since the last job description of record. Instructions for completing the PD are available below. 
Who Should Use the Form: The form should be initiated by a supervisor.
