| Purpose: |
The U.S. government requires UCAR to certify that it has reviewed the EAR and ITAR and determined whether it will require a U.S. government export license to release controlled technology or technical data to a prospective employee who is a foreign national. UCAR must now indicate whether or not a license is required when filing any H-1B visa application on Part 6 of Form I-129. The determination made on this document will be transferred to form I-129, which is filed with the United States Citizenship and Immigration Services. |
| Who Should Use the Form: |
When an H-1B visa application is being filed, this form is required to be completed by the prospective or current employee’s lab/division/program. The Division Administrator or Supervisor/Host as well as the EO or ECC must sign and complete the form. The H-1B visa application cannot be filed until this form is completed. |
| Instructions: |
Check the proper box to indicate whether an export license is required. Fill in the name of the foreign national for whom the H-1B application is being filled on behalf of. Obtain signatures from the Division Administrator or Supervisor/Host and EO or ECC. Once complete, return the form to Shonna Montoya in Human Resources, who will use the information to complete the I-129 H-1B application and will keep a copy in the H-1B visa file. |
