Purpose:	The Employee Tax Certification form is used when determining whether Educational Assistance Program courses are taxable or non-taxable per Internal Revenue Code.
Who Should Use the Form:	When applying for Educational Assistance Program courses, the employee must complete the Employee Tax Certification form.
Instructions:	The employee must complete Parts 2 and 3 and submit it along with their completed Educational Assistance Program Application form to Human Resources.
