Purpose: This form is used to make any changes that affect an employee's account keys and percent allocations.
Who Should Use the Form: Division/Program Administrator
Instructions: Complete form, obtain appropriate signature(s), submit to Human Resources for processing. Once Administrators have access to iVantage to make their own changes, this can be used as a worksheet or perform the directly into the iVantage system. View the form for complete instructions.
